Providing Pre-admission Screening, Advocacy, & Service Coordination for Individuals with Intellectual Disabilities
WISC provides services for individuals with intellectual disabilities
living mainly within the 7 counties served. Occasionally we serve individuals receiving ISSA services living outside of our service area based on the individual's choice of ISC agencies. This is determined on a case by case basis. If you are in need of services now or within the next 5 years, please contact WISC. Services Provided by WISC
*PUNS (Prioritization of Urgency of Need for Services): Statewide database/waiting list for services for people with intellectual disabilities who are potentially in need of services now or within the next 5 years; Contacting your local ISC agency to get on PUNS is the first step to take in accessing services; The PUNS is to be updated at least annually or anytime there is a significant change in the individual's situation.
*Pre-Admission Screening (PAS): WISC arranges for and completes assessments to determine eligibility for Medicaid waiver funded services, makes necessary determinations regarding eligibility for services, educates individuals and families, makes referrals and provide linkage to appropriate and needed services;
*Individual Service & Support Advocacy (ISSA): Completes Discovery Tool and Personal Plan for each individual and completes at a minimum one followup visit per year; provides advocacy services for the individual, offers choice, assures rights are protected, and enhances the delivery and effectiveness of service delivery through person-centered planning;
*Bogard Individual Service Coordination (ISC): coordinates the individual service plan, completes monthly visits to Bogard class members, offers choice, provides advocacy, service facilitation and broker Bogard specialized services;
*Community Independent Service Coordination: assists individuals with the PUNS process;
